📌 Please Note:
– Minors must be accompanied by an adult
– MCF is not responsible for participant safety or personal belongings
– Be sure to sign all required waivers on the Aquatic Center website before the event

Rental Info (per Sacramento State Aquatic Center):
	•	Rentals are first come, first serve
	•	Everyone 18+ must bring a photo ID (will be held during your rental)
	•	Each group must leave one set of car keys as a deposit
	•	All participants must sign a Participant Waiver
	•	Those 18+ must also sign the Equipment Rental Terms
	•	Minors must have waivers signed by a parent/guardian beforehand
	•	All rentals must be off the water by the posted closing time

(Initial) Waiver and Release  I hereby release and hold harmless the Muslim Community of Folsom (MCF), its board members, volunteers, employees, affiliates, partners, event organizers, and all associated individuals from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, accident, illness, loss, or property damage that may occur during or as a result of participation in MCF events.  This release applies even if such injury or loss is caused in whole or in part by the negligence or other fault of MCF or its agents.

(Initial) Acknowledgment of Risk and Responsibility  I acknowledge that participation in any event organized by the Muslim Community of Folsom (MCF) may involve activities that carry some risk of personal injury, property damage, or other loss. These risks may arise from, but are not limited to, physical activity, transportation, weather conditions, food consumption, equipment use, and the actions or negligence of others.  By signing this form, I affirm the following:  I voluntarily choose to participate (or allow my child/ward to participate) in MCF events.  I understand and accept the inherent risks involved.  I accept full responsibility for my (or my child’s/ward’s) conduct and actions during the event.  I certify that I (or my child/ward) am physically and mentally able to participate in the event(s).

(Initial) Photo/Video Consent (Optional) I grant MCF permission to use photos or videos taken of me (or my child/ward) at events for promotional, informational, or educational purposes.
