Medical Fitness I certify that I am physically capable of participating in these activities and have no medical condition that would make participation unsafe. I understand that I am responsible for monitoring my own health and fitness.  Emergency Medical Care In the event of an emergency, I authorize 626 Matcha Run Club organizers to obtain medical assistance on my behalf if I am unable to do so. I understand that I am responsible for any resulting medical expenses. Photo & Video Release I grant 626 Matcha Run Club permission to photograph, record, and use my image, likeness, and voice in promotional materials, social media content, and other club-related communications without compensation.  Stroller Participation If I choose to run with a stroller, I assume all associated risks and agree to exercise extra caution, maintain control of the stroller at all times, and remain toward the back of the group when appropriate.  Dog Participation If I choose to bring a dog, I accept full responsibility for my dog's behavior and safety. I agree to keep my dog on a short leash, under control at all times, and to comply with all applicable local laws. I am solely responsible for any injury, damage, or claim arising from my dog's actions.  By checking this box, I acknowledge that I have read and agree to the terms of this waiver and release.
