•‿•Expect a night filled with great vibes, new friends, and a space made just for you—by you. Dance the night away to an amazing DJ, grab some delicious tacos, and explore different ways to express yourself. Make your own feathered masquerade mask, get a henna tattoo, or challenge a friend to a round of air hockey in a chill lounge area or play video games on Switch with out big projector! Whether you want to tear up the dance floor or just hang out in a space where you can be fully yourself, this prom is yours. A night to celebrate, connect, and shine—surrounded by other queer Jewish teens who get it!!
Waivers & Agreements
JQ International in-person events are open to individuals who have been vaccinated for COVID-19 or are medically unable to be vaccinated. By purchasing a ticket to this event, you are hearby confirming that you:
1.) are fully vaccinated, meaning it has been 2 weeks after your second dose of the Pfizer-BioNTech or Moderna COVID-19 vaccines, or 2 weeks after your single-dose Johnson & Johnson's Janssen COVID-19 vaccine.
2.) are medically unable to be vaccinated, and agree to wear a proper face mask covering when attending JQ International events.
In consideration of my participation in the foregoing, the undersigned acknowledge and agree to the following:
I am aware of the existence of the risk on my physical appearance to the venue and my participation to the activity of this event that may cause injury or illness such as, but not limited to Influenza, MRSA, or COVID-19 that may lead to paralysis or death.
I have not experienced symptoms that of fever, fatigue, difficulty in breathing, or dry cough or exhibiting any other symptoms relating to COVID-19 or any communicable disease within the last 14 days.
I have not been, nor any member(s) of my household, diagnosed to be infected of COVID-19 virus within the last 30 days.
Following the pronouncements above I hereby declare the following:
I am fully and personally responsible for my own safety and actions while and during may participation and I recognize that I may be in any case be at risk of contracting COVID-19.
With full knowledge of the risks involved, I hereby release, waive, discharge JQ International from any and all liabilities, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, injury, or death, that may be sustained by me related to COVID-19 while participating in any activity while in, on, or around the premises or while using the facilities that may lead to unintentional exposure or harm due to COVID-19.
I agree to indemnify, defend, and hold harmless JQ International from and against any and all costs, expenses, damages, lawsuits, and/or liabilities or claims arising whether directly or indirectly from or related to any and all claims made by or against any of the released party due to injury, loss, or death from or related to COVID-19.
By submitting this form, I acknowledge that I have read the foregoing Liability Release Waiver and understand its contents; that I am fully competent to give my consent; That I have been sufficiently informed of the risks involved and give my voluntary consent in signing it as my own free act and deed; that I give my voluntary consent in submitting this form that includes this Liability Release Waiver as my own free act and deed with full intention to be bound by the same, and free from any inducement or representation. This waiver will remain effective until laws and mandates relevant to COVID-19 are lifted.

Photo Release Agreement
When attending this retreat, JQ International seeks permission to use, re-use, publish, re-publish, and modify or alter images taken during events. These images may be used for editorial, commercial, trade, advertising, and other purposes in any medium now existing or developed in the future, including but not limited to the JQ International, Keshet, and Eden Village West websites and other online platforms. This usage extends worldwide and in perpetuity for the purposes stated above.
By attending this event, you grant permission for JQ International, Keshet, and Eden Village West to use your image and release them from any and all claims arising from such use, including but not limited to claims for libel, invasion of privacy, or other related concerns.
If an attendee does not agree to this policy, when possible, JQ will provide a special sticker to indicate that their photo should not be taken and/or used for marketing purposes. It is the attendee’s responsibility to notify JQ of this preference.

Liability and Indemnity Waiver
My teen is physically and emotionally fit enough to participate in the Program. In case of emergency, I hereby give permission to JQ International and medical personnel to provide my child with urgent or emergency health care; to administer anesthesia and medications; to order X-rays, tests, and provide treatment including surgical treatment; to release any records necessary for insurance purposes; and to provide or arrange necessary related transportation for my child. In the event I cannot be reached in an emergency, I hereby give permission to the physician to secure and administer treatment, including hospitalization, for the teen named above. I understand that every effort will be made to contact the parent/guardian or the name listed as 'Emergency Contact' I further understand that if I do not have medical insurance that covers all costs, I will be responsible for such medical costs.
In consideration of being permitted to participate in the above-referenced Teen JQSA (the “Program”), the undersigned Teen and the undersigned Teen’s parent or legal guardian on behalf of Teen and for himself or herself or themself and on behalf of all of Teen’s parents, legal guardians, representatives, heirs, and next of kin, hereby agrees to the following:
THE UNDERSIGNED HEREBY RELEASES, WAIVES, DISCHARGES AND COVENANTS NOT TO SUE JQ INTERNATIONAL, its officers, employees, directors, advisors, contractors, consultants, and agents (hereinafter referred to as “releases”) from all liability to the undersigned, the undersigned’s personal representatives, assigns, heirs, and next of kin for any loss or damage, and any claim or demands therefore on account of injury to the person or property or resulting in death of the undersigned, while the undersigned participates in the Program, waiving liability from any and all claims including the negligence of JQ International, its related organization, its officers, employees, and agents, resulting in personal injury, accidents or illnesses (including death), and property loss arising from, but not limited to the activities and attendance at JQ International Events events and programs.
THE UNDERSIGNED HEREBY AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the releases and each of them from any loss, liability, damage or cost they may incur due to the participation of the undersigned in the Program.
THE UNDERSIGNED HEREBY ASSUMES FULL RESPONSIBILITY FOR AND RISK OF BODILY INJURY, DEATH OR PROPERTY DAMAGE while participating in the Program.
Agreement to attend this event is agreement to the above parameters.
